Program Description
CHRC is an intensive individualized program requiring the client's and (when applicable) family's participation.
Most of the participation occurs during program groups. These group meetings include education, therapy and support.
Each client is
assigned a primary case manager who conducts an assessment using the Addiction Severity Index tool. The case manager and client then
corroborate to develop an individualized treatment plan based on information obtained during the assessment. Throughout the treatment
episode the case manager works with the client's specific individual issues and reviews his/her program assignments. This provides
for ongoing assessment of progress and each client's progress is evaluated in a treatment team meeting. An individualized exit plan
is developed for each client to include living arrangements; employment and/or education; aftercare and continuing recovery efforts.
Each client attends aftercare one time weekly for a period of 6 months following discharge. Throughout the program, even after completion
of the formal program, clients are encouraged to continue their 12-step recovery work. This includes regular attendance at AA, NA,
CA, and DDA meetings, as well as working with their sponsors.
